Atlanta Medical Institute describes itself as a cash-pay practice and says it provides superbills. Confirm the arrangements for your visit with the office. A superbill is documentation you may submit to a health plan; reimbursement depends on that plan’s rules.
A cash-pay office visit does not establish whether your pharmacy benefit covers a prescription. Ask the insurer about the exact drug, indication, formulation, pharmacy, deductible, and authorization criteria. FDA approval does not itself require your plan to pay for a product.
Before starting a GLP-1 medication
Request the current formulary and coverage criteria directly from your plan. Record the date and reference number for the conversation. Ask whether obesity treatment is a covered benefit, which documentation the prescriber needs, and when approval must be renewed.
Do not assume that a diabetes medication is covered for weight management because it shares an ingredient with a weight-management brand. Likewise, a manufacturer savings offer may have eligibility restrictions and an expiration date. Confirm terms through the manufacturer and insurer.
If coverage is denied
Obtain the written denial and identify whether it concerns a benefit exclusion, missing documentation, prior authorization, or another issue. Ask the plan about review or appeal instructions and deadlines, and give the notice to your treating clinician. An appeal does not guarantee approval. If care becomes unaffordable, discuss alternatives before changing treatment yourself.
