When you use a contact form, the site sends your name, email address, phone number, and message to the shared clinic platform so authorized staff can respond. The submission records which website received the inquiry. Please do not use general contact forms for detailed medical records, emergencies, or highly sensitive information.
Appointments and the patient portal
Portal and booking buttons take you to the clinic’s shared patient platform at amipcaportal.com. Your patient account is shared across participating clinic brands; choosing a website helps route your care and does not create a separate patient profile. The portal provides its own account, privacy, payment, and consent information.
Technical information
Website hosting and security services may process technical information such as request time, IP address, browser information, and errors to deliver and protect the service. Necessary preferences may be stored by the browser. External websites, including maps and video platforms, apply their own privacy policies when you open them.
Website analytics
We use Google Analytics on our public website to understand visits, page views, and how visitors use the site. Google Analytics may use cookies and collect browser, device, and usage information. Our analytics integration does not send contact-form field values to Google, and Google signals and advertising personalization are disabled in our tag configuration. You can manage cookies through your browser settings.
